Health Status Statement

My temperature is UNDER 100 degrees Fahrenheit.

I am NOT experiencing ANY of the following:  cough (new or worsening), shortness of breath (new or worsening), troubled breathing (new or worsening), fever, chills, muscle pain (new or worsening), headache (new or worsening), sore throat (new or worsening), new loss of taste or new loss of smell.

I have NOT tested positive for COVID-19 through a diagnostic test in the past 14 days nor do I reside with anyone who has tested positive within the last 14 days.

I have NOT had any known contact with a suspected or confirmed case of COVID-19 in the past 14 days.

I will notify the SOTC President or member of the SOTC  Board, or my instructor and assistant instructor, IMMEDIATELY if I feel ill with any of the above, either during the time I am in the building or any time after.

I will abide by all social distancing guidelines upon entering the premises of SOTC.  This includes parking lots, hallways, bathrooms and training area.
